EHCP Support

An EHCP stands for an Education, Health and Care Plan.

EHCPs are awarded to children and young people who, as a result of their additional challenges, warrant extra nurturing, support and monitoring as they progress through school. The assessment for these plans, and the monitoring and reviews afterwards, rely on specialist advice being provided. They are designed to safeguard all aspects of a child’s journey through education.

An EHCP is a statutory document that is written and coordinated by a Local Authority, in collaboration with your child’s school or college, and a range of other professionals. If appropriate, it can stay in place until the young person reaches the age of 25.

Not all children’s needs meet the criteria for an EHCP, but if challenges have been identified then the child or young person will likely be on the school’s SEN register.

Education, Health and Care Plan (EHCP) Outcomes & Goal Setting

For children we already know, we are sometimes asked to provide Outcomes which will form part of the EHCP statutory document. 

EHCP Outcomes include measurable goals for the forthcoming year or years, alongside what is needed to be in place to meet them. They describe the progress that a child or young person will make as they progress through school or college.

We can provide Outcomes and Goals and choosing us to do so, allows us to spend time with your child and you to ensure their voice is captured in the advice we offer. 

If it has been a while since we carried out our assessment, a reassessment may be recommended so that up to date evidence can be submitted.

EHCP statutory visits 

We support many children who have been awarded an EHCP. This means that we can visit your child or young person at school or college, at regular points throughout the academic year, to be able to monitor the progress they are making and review the targets and support that has been recommended.

EHCP Outcomes and Goals require careful monitoring which, if included in the EHCP, becomes part of a statutory role. 

EHCP Outcomes as a bolt-on to any assessment or evaluation

If we are asked to provide EHCP Outcomes at the time of our assessment or evaluation then an additional hour is charged. 

We need to know this before we carry out the assessment.

Attendance at an EHCP Annual Review Meeting

If your child or young person is on our caseload then we will not only be able to provide the statutory monitoring visits through the year, but will also be able to attend to the annual review of their EHCP.

This means we can be by your side at the Annual Review Meeting and contribute our advice and recommendations alongside all those present. The Annual Review is coordinated by the school or college SENCO, so he or she will reflect our contribution in the paperwork they then submit to the Local Authority

Attendance at Team Around the Child (TAC) or Team Around the Family (TAF) meetings  

TAC or TAF meetings can take place for any child or young person on the setting’s SEN register, whether they have an EHCP or not.

These meetings create an opportunity for a child or family centered discussion about recent events, progress or planning. If specialist support at a meeting is needed then you or the SENCO can ask us to attend.
